Cell Phone Reimbursement
Take advantage of this opportunity to receive a $5 reimbursement per pay period for your cell phone plan! To enroll, you will need to turn in a copy of your cell phone plan to Human Resources and bill each pay period. Reimbursements are given to subsidize the usage of text messaging of TIFFE notices. Sprint providers are unable to receive this reimbursement.

Driver Data Updates
In order to keep your personnel/credential file current, please submit a copy of your driver's license, no-fault insurance or car registration upon update directly to the Human Resources Department. To ensure your documents don't get lost, please do not submit to any other department / staff / supervisor.

Get Involved With TIFFE's 401(k)
401(k) Match, Vesting Schedule Change

The Company has elected to start an employer match to the 401(k) Retirement Savings Plan, effective 7/1/2004. The company will match fifty cents for each dollar you contribute up to 1% of your annual compensation. The company has the discretion to match more than 1% of your annual compensation, which will be decided after the plan year ends.

In order to be eligible for the matching contribution, you must (a) work at least 1,000 hours in the calendar year, (b) still be employed by the Company on the last day of the calendar year, and (c) have already reached your “eligibility date”. Your “eligibility date” is the 1st day of January or July next following the day on which you complete a “year of service”. A year of service means a 12-month period during in which you worked at least 1,000 hours.

The vesting schedule of the Plan is also changed, effective 7/1/04. The new vesting schedule will not affect participants who are fully vested or are already eligible to make 401(k) salary reduction contributions by 7/1/2004 (i.e., an employee who regularly works 20 hours per week and has been with the Company for at least 3 consecutive months. i.e., any employee hired before April 1st, 2004). The new vesting schedule is as follows:

1 year of service = 25% vested
2 years of service = 50% vested
3 years of service = 75% vested
4 years of service = 100% vested

How do I enroll in medical?
Employees must first work 20 hours a week for four (4) consecutive weeks to be eligible to receive medical benefits. Decide which available medical provider you would like to go with (HMSA, UHA or Kaiser - see medical memo) and then you must come to TIFFE's Human Resources Department to fill out the necessary paperwork.

How do I enroll in 401(k)?
Upon reaching 6 months of employment, you will automatically be sent a packet regarding enrolling in TIFFE's 401(k) through American Funds. You will also be sent a personal identification number (PIN) for your account - do not lose this number! Enrollment is done online (www.myretirement.americanfunds.com) or on the phone (800-204-3731) through American Funds.

How do I change my address?
Upon knowledge of an address change, immediately notify the Human Resources Department of the change by completing the Change Of Address Form and submitting it to HR.

How do I get a new TIFFE badge?
Simply notify the Human Resources Department that you will need a new badge and come to HR to sign your new badge. Upon completion of your badge, your badge will be placed in your TIFFE inbox.

How do I request time off?
To request time off, you will need to fill out a Leave Request Form and have your supervisor sign it for approval. Upon approval, you then need to find a substitute for your cases and notify the Intake Department regarding your need for a substitute. If you are unable to find a substitute, notify the SubLine immediately at (808)748-3163.

What do I do if I get hurt on the job?
Report all injuries to your supervisor and the Human Resources Department immediately! Then completely fill out an Incident Report within 24 hours to your supervisor and HR to be considered for Workers' Compensation Insurance. If injury requires a doctor's examination, please notify HR.

Where can I find all of TIFFE's Policies and Procedures?
A binder of all of TIFFE's Policies and Procedures can be found in several locations:
    - At the main reception desk in Room 105 (Main Office)
    - On top of filing cabinets on left upon entering Room 106 (Main Office)
    - In the Human Resources Department (Main Office)
    - Each TIFFE office location
